CHAPS Building, MCPS-Medicaid Managed Care AdminCITY:
The Population Health Specialist will report to the Cone Health Medical Group and Ambulatory Services (CHMG/AMB) Director of Quality and collaborate with CHMG/AMB physicians, mid-level providers, staff and other health care professionals to promote quality improvement efforts, performance, and reporting across the health care continuum.
The primary focus will include:
1- Identifying and working with high risk patient populations (including high utilizers).
2- Working to ensure CHMG metrics are being met across the division.
3- Providing oversight to quality improvement initiatives as directed.Required:
Must have working knowledge of computer databases, spreadsheets and wordprocessing software; be able to visualize the computer screen and interpretinformation on the screen.
Demonstrate computer skills to utilize electronic medical record.Data Entry - intermediate knowledge
Knowledge of basic disease prevention tests and procedures.
Skills to establishing and maintaining effective working relationships withpatients, medical staff, and the public.
Cultural sensitivity to diverse populations.
Ability to evaluate and appropriately respond to verbal and nonverbalcommunication from patient's divers stages of development, i.e. pediatrics,adolescents, adults and geriatrics.
Skills in preparing and maintaining records, writing reports, and responding tocorrespondence.
Proficiency with Microsoft OfficeEDUCATION:
High School Diploma or equivalent, Required, and Graduation from an accreditedMedical Assistant programRequired:
Minimum 1 Year of ExperiencePreferred:
1 years of experience working in a Primary Care or Public Health settingRequired:
