LOCATION: Alamance RegionalMedical Center, SW-Care Management
CITY: Burlington
JOB DETAILS:
Exempt: Yes | FTE: 1.0 (40 hours/week) | Schedule: Monday - Friday 8 am - 5 pmoccasional Saturday | On Call: YES
JOB SUMMARY:
Monitors patient care progresstoward goals, makes recommendations and/or utilizes appropriate resources tooptimize effective, efficient care progression and care plan goal achievement.Coordinates care transition from inpatient to next level of care. Performsadmission and continued stay utilization reviews and discharge screening toassure the necessity of hospital admission, appropriate level of care,continued stay and supportive services, appropriate safe discharge/transitionplan, and to examine delays in the provision of services. Collaborates withattending Physicians, Mid-level Providers or Department Medical Directorsconcerning status and/or medical necessity issues. When criteria is inquestion, escalates care for second level Physician review when DepartmentDirector is unavailable.
EDUCATION:
Required: Bachelors, Nursing
Preferred: Masters, HealthcareRelated Field
EXPERIENCE:
Required:3-5 years of recent related acute care experience
Preferred: 1-2 years of CaseManagement and/or Utilization Review experience
LICENSURE/CERTIFICATION/REGISTRY/LISTING:
Required:
RN | Registered Nurse licensedin North Carolina or a Compact state.
- Current North CarolinaRegistered Nurse license required.
- CPR is required for CaseManagers at Behavioral Health locations and is preferred at all otherlocations.
Preferred:
- Current Case Managementcertification of CCM or ACM preferred.

- CPR is required for CaseManagers at Behavioral Health locations and is preferred at all otherlocations.
