The Oncology Hospitalist APP will facilitate care of hospitalized new consults and ongoing continuity of care for hospitalized oncology patients.
They will be a member of an integrated team of providers committed to top level, evidence based, patient focused care.EDUCATION:
Graduation from an accredited Physician's Assistant or Nurse Practitioner's program.EXPERIENCE:
Heme/Onc hospitalist experience Preferred.
2 years of hospitalist, internal medicine or medical specialty experience may be helpful.
New graduates will not be considered.LICENSURE/CERTIFICATION/REGISTRY/LISTING:
BLS (CPR) | AHA Health Care Provider BLS
Current PA or NP licensure in the State of NC
Preferred: ACLS certificationESSENTIAL JOB FUNCTIONS:
Performs daily rounds on oncology patients admitted to the hospital. Consistently performs daily rounds in such a manner that meets or exceeds patient care needs and physician satisfaction.
Responds to critical or emergency care alerts within the hospital units. Consistently responds to emergent and critical alerts. Is responsive to supervising physician directives and orders, allowing for least amount of disruption to daily rounds.
Ensures continuity of care for patients, maintaining communication with the Medical Oncologists. Manages requests and notifications by pager and by phone, including necessary new orders for medications and other patient needs. Consistently coordinates patient care and manage communications in such a manner that meets or exceeds patient care needs.
Serves as the first responder for any new Hem-Onc consults from hospitalists. Performs initial assessment and plan. Communicates with the on-call Medical Oncologist in a timely manner. Coordinates their care with the Medical Oncologist and referring physicians. Consistently performs complete and accurate patient assessments that require little to no physician rework. Consistently assesses patients in such a manner that meets or exceeds patient care needs.
Reviews lab results, x-rays, and other test results with physicians, and organizes information for rounds. Consistently performs complete and accurate review of patient results. Consistently reviews and communicates results in such a manner that meets or exceeds patient care needs.
Under the guidelines of state and federal licensing, and according to hospital policies, performs patient procedures as ordered by the physicians. Consistently performs patient procedures with skill in such a manner that meets or exceeds patient care needs.
Serves as a point of contact for inpatient activities for nursing staff and physicians regarding admissions and discharges. The APP will complete daily evaluations of all oncology patients and enter appropriate orders (medications, labs and imaging) to optimize the patient's clinical progress. Documentation of the daily patient visit will be focused but complete, covering inpatient clinical issues in a clear, concise format. New patient consultations will be completed by the APP at the request of the Medical Oncologist. Consistently coordinates admissions and discharges and manages communications in such a manner that meets or exceeds patient care needs. Accurate discharge documentation is especially important for a successful, safe transition from the hospital. Actions have a positive impact on patient LOS.
The APP and Medical Oncologist work together to provide optimal care to every patient. The importance of effective communication of the patient's needs to the Medical Oncologist is vital. The APP works throughout the hospital stay to prepare the patients for discharge through patient and family education, emotional support and carefully completed discharge prescriptions and appointments.
Participates in cross coverage sign outs with physicians when appropriate. Consistently coordinates and communicates between shifts in such a manner that meets or exceeds patient care needs.
Office/Clinic Evaluation - The APP is available to assist Medical Oncologists with caring for clinic patients as needed. Some patients requiring hospital admission from the clinic will need orders as well as history/physical documentation.
Assists in reviewing and collecting data for use in quality improvement and for monitoring outcomes. Consistently provides support in review and collection of data. Responds to requests for data as needed.
Participates in professional development activities and maintain professional affiliations. Actively participates in appropriate tumor boards, staff meetings, committees and training as requested. Maintains information in a confidential manner in accordance with State, Federal and Practice guidelines.CONDITIONS OF EMPLOYMENT:
