LOCATION: CHAPS Building, THNM-THN CM Community
CITY: Greensboro

JOB DETAILS:
Exempt: Yes | FTE: 1.0 (40 hours/week) | Schedule: Monday - Friday 8am - 5pm | On Call: NO

JOB SUMMARY:
Provide telephonic care management/ coordination services and focused disease education to a contracted community based high-risk populations. Contracted populations can include members of the following: Medicare Advantage Plans, Physician Provider Networks, and Employer Plans. Goal is to promote quality coordinated cost-effective outcomes for an assigned caseload. Performs telephonic care coordination and on-going clinical and psychosocial assessments to identify needs and monitor client progress. Assessments, health education and chronic disease management are performed by telephone at the THN Care Management office. Individualized plans of care are developed to facilitate self-monitoring and self-management skills of a health condition and improve function and quality of life for members. Care Coordinator will be an advocate for appropriate resources available in the community, and across the continuum of care to best meet the needs of the client. This role will interact with all levels of personnel, medical staff, patients, community resources, providers and families.


EDUCATION:
Required:
Acquired degree as an RN

Preferred:
BSN or Advanced Degree in Nursing


EXPERIENCE:
Required: Five years nursing related care experience and/ or home care experience combined.
Preferred:
Case Management, Care Management, Telephonic Case Management, and/or Disease Management experience


LICENSURE/CERTIFICATION/REGISTRY/LISTING:
REQUIRED
RN | Registered Nurse licensed in North Carolina or a Compact state.
Drivers License | Valid Drivers License
BLS (CPR) | AHA Health Care Provider BLS


PREFERRED
Case Management Certification
