Exempt: Yes | FTE: 1.0 (40 hours/week) | Schedule: Monday-Friday, Days | On Call: No

JOB SUMMARY:

Provide telephonic chronic disease management services to a contracted community based high-risk population. Contracted population currently includes members of the following: Medicare Advantage Plans and Physician Provider Networks. The goal is to promote improved quality of life by patient centric care through education, member monitoring and self-management promotion. The RN Health Coach accurately conducts a thorough, objective, holistic, clinical, psychosocial, physical, and educational needs evaluation of the patients' current disease state. Develops and implements an individualized plan of care. Maintains current knowledge of disease processes, available resources, and treatment options appropriate to patient population. Consistently documents all activities according to policy in the electronic medical record. Continuously demonstrates a positive attitude and understanding of disease management, supports team building, and is motivated to fulfill department objectives. Maintains ongoing communication of the care plan and timeline to the member and provider. Consistently demonstrates excellent communication and interpersonal skills. RN Health Coach will interact with the interdisciplinary team to include but not limited to: medical staff, patients, providers, families and caregivers. Assessments, health education and chronic disease management are performed by telephone at the THN Care Management office.

EDUCATION:
BSN or Advanced Degree in Nursing preferred.

EXPERIENCE:

5 Years Recent Nursing Experience, Required

Required:
Five years recent nursing experience.
Clinical knowledge and skill in the application of the techniques and practices of the nursing profession.
Ability to understand and implement disease management philosophy and principles.
Ability to explain medical instruction to members and their families.
Prior experience and ability to educate clients of all ages about the following core disease management issues including but not limited to: Diabetes, Heart Failure, Hypertension, Hyperlipidemia, CAD, Asthma, and COPD required.
Experience and ability to use Microsoft Office products and word-processing software on a daily basis.

Preferred:
Case Management, Telephonic Case Management and or Disease Management experience preferred.

LICENSURE/CERTIFICATION/REGISTRY/LISTING:

Required:

RN | Registered Nurse licensed in North Carolina or a Compact state.
