JOB SUMMARY:
Assesses, plans, implements, documents, organizes, prioritize, delegates, supervises and coordinates the care
of neonatal, pediatric, adolescent, adult, geriatric patients as prescribed within the parameters of the NC Nurse Practice Act and hospital complex.

Associates Nursing Required
Bachelors Nursing Preferred
Additional Information: Graduate of a School of Nursing. BSN required or with approval of the hiring manager, experienced RN's with an AD or Diploma with an active plan to obtain their BSN within four years from date of hire may be considered.

Scope of Services
Department
2H/ Cardiovascular Intensive Care(CVICU) Unit

1. Department Description - 2 HeartCVICU is a 26 bed patient care department with 26 private, hard-wired monitoredbeds with telemetry capability that provides care primarily tocardiovascular/thoracic intensive care patients, 7 days a week, 24 hours a day.The patient rooms are located on the perimeter of a rectangular shapeddepartment with 7 nursing stations around the department. Supply storage is in between the nursingstations in the center of the department. Each nursing station contains apatient call system and 3 nurse stations with central cardiac monitoring systemat the center and at each end of the department.

2. Population/ClientsServed - High volume patientpopulations include those experiencing an acute myocardial infarction (MI) withcomplications, complex arrhythmias, heart failure, undergoing Cardiac Surgery,Aortic Abdominal Aneurysm Repair, Thoracic Abdominal Aneurysm Repair,Esophagectomy patients, Thoracotomy, multiple trauma, sepsis, Shock, acuterespiratory failure, renal failure, hypertensive crisis, and CVVH. Low volumepopulations include head trauma, stroke, pneumonia, drug overdose, diabeticketoacidosis, GI bleed, and long term ventilator patients. The average length of stay is 34.71 daysbased on admissions. Age groups served are adult and geriatric.

3. ServicesProvided - Nursing care is provided24 hours a day, 7 days a week by an RN staff that is supported by ancillarystaff of Nurse Technicians and a Nursing Secretary. Nursing care includescardiac and respiratory assessment and monitoring, pharmacological support tothe hemodynamically unstable patient and medical and nursing interventions toachieve physiologic stability and prevent complications. Services also includepatient education related to the surgical procedure, disease process, other procedures,and support services for diagnostic purposes and rehabilitation.
The medicaland nursing directors are responsible for triage of patient admissions. Theyare supported by and collaborate with the Assistant Director (AD), Charge Nurse(CN) and RN IV. Triage is determined by using admission and discharge criteriaapproved by the Critical Care Committee.

4. Staffing Plan/Ratios - Theavailability of staff to provide services

RegisteredNurses, Nursing Technicians, and Nursing Secretary. Registered Nurses, Nursing Technicians arescheduled 7 days a week, 24 hours a day and Nursing Secretary are scheduled 7days a week, 16 hours a day. Patient assignments are based upon patient acuityand staff skill. We budget for a ratio of 1.72 patients per RN 24 hours a day.The goal is two Nursing Technician each shift and one Nursing Secretary from0700 to 2330 each day.

Staff arescheduled for 6-week periods and distributed based upon anticipated daily census, shift census, and staff experience.

5. Competency/EducationPlan - Orientation/InitialCompetency - Each role has a defined orientation pathway or competencyvalidation list that is completed prior to the person taking an independentassignment. Key competencies are validated by direct observation by theindividual preceptor or experienced staff. The Department Director determines,with input from the preceptor(s) and by observation, when the orientation iscomplete.

Ongoing Competency - Staff member'songoing competency to provide care on CVICU is determined by direct observationand record review by the Department Director, Assistant Director, Educators,and Shift Leadership. Competency is maintained by skills fairs, routine reviewsof low volume and problem-prone procedures, and EKG revalidation.

ACLS is required for all RNs workingin the following departments across the MCHS: PACU, outpatient surgery, adultcritical care, step-down, telemetry departments in which standing emergencyorders are utilized, maternity admissions unit, emergency departments, andEndoscopy. ACLS is required for family practice and internal medicineresidents, Carelink staff, Administrative Coordinators, Cardiac cath lab techsand cardiac rehab RNs.

Cone Health RN Weekend OptionProgram
1. Scheduleand FTE Status Options: The following schedule and FTE options are available:
EverySaturday - Sunday .6 FTE
EverySaturday - Sunday plus one extra 12 hour shift in the pay period .75 FTE
EverySaturday - Sunday plus two extra 12 hour shifts in the pay period .9 FTE
2. Weekend Option Premium: Employeeswill receive base pay plus a $5.00 weekend option premium for hours worked upto their FTE commitment each pay period. Hours worked above FTEcommitment will be paid at base rate plus shift differentials and overtime ifapplicable, but not including the $5.00 weekend option premium.
3. Weekend Option Allowance: Employeeswill be eligible for a Weekend Option Allowance equivalent to 6.5 paid weeksoff based on FTE every fiscal year. This Weekend Option Allowance willinclude the $5.00 WeekendOption Premium and will be available for use each October 1 as follows:
RN WeekendOption 48 (.6 FTE) 156 Weekend Hours
RN WeekendOption 60 (.75 FTE) 195 Hours(156 Weekend / 39 Weekday)
RN WeekendOption 72 (.9 FTE) 234 Hours (156 Weekend / 78 Weekday)
Weekend OptionAllowance hours will be pro-rated for employees entering the plan after October1st. The full or pro-rated allotment is front-loaded and available foruse immediately. Any unused Weekend Option Allowance hours are forfeited at theend of the fiscal year or if the employee leaves employment with Cone Health ormoves out of this Weekend Option Program.
4. Use of Weekend Option Allowance: Front-loadedWeekend Option Allowance will be available upon start date in the program andmay be used in any sequence and at any time with Department Director approval. Refer to departmental process for vacation/time off requests.
5. Unscheduled Absences: Inthe event of an unscheduled absence, the employee is expected to make theirtime up in the current week (or pay period, if no overtime is incurred), orthey may use the time allotted for their weekend option allowance. Unscheduled time off, even if made up, will be managed according to theCone Health attendance policy. Unscheduled absences can only be made upwithin the current pay period.
6. PAL Eligibility: Because employees in theWeekend Option Program are eligible for a generous Weekend Option Allowance,they are not eligible for traditional PAL (Paid Annual Leave) or CatastrophicSick Bank (if applicable).
7. Call-Offs: RNs in this Program will be the lastto be called off for weekend shifts. Weekday call-offs will follow normaldepartmental scheduling guidelines.
8. Benefits: Benefits are based on the employee'sFull Time Equivalent (FTE). Employees holding an FTE status of .60 willreceive Cone Health's benefit package for part-time employees; those with FTEstatuses of .75 or .9 will receive full-time benefits.
9. Holidays: Holidays that fall on a scheduledshift will be eligible for Holiday premium in accordance with HR PolicyER-HRD-2005-29, Holidays. Holiday Premium is 50% of the employee's baserate of pay.
10. Differentials and Special Incentives: Employeesin the RN Weekend Option Program are eligible for off shift differential butnot weekend differential. Weekend Option staff will be eligible for incentivebonuses, if offered.
