JOB SUMMARY<\/b>:

Assesses, plans, implements, documents, organizes, prioritizes, delegates, supervises and <\/font>coordinates the care <\/span>of adolescent, adult, geriatric patients as prescribed within the parameters of the NC Nurse Practice <\/span>Act and <\/span>hospital complex.<\/span><\/p>

RN - Registered Nurse licensed in North Carolina or a Compact state.<\/span>
BLS (CPR) - AHA Health Care Provider BLS<\/span><\/span><\/p>

Certification in a clinical specialty as defined by National Certification requirements is required within o<\/font>ne (1) year into Registered Nurse 3 position.<\/span><\/p>

MAJOR WORK ACTIVITIES<\/b>:
<\/font><\/span><\/p>

Provides nursing care via the nursing process and data analysis for the development, <\/font>implementation and evaluation of nursing care. Interviews patient to obtain pertinent <\/span>medical and surgical history, current medications, treatment plan and preparation for <\/span>examination. Performs age specific assessments to collect pertinent data. Identifies <\/span>needs based on knowledge of growth and development of adolescents, adults and <\/span>geriatric patients, records initial database and ongoing assessment pre-, intra-, and <\/span>post-procedure. Consistently recognizes and acts upon subtle changes in patient <\/span>status, anticipating their needs while collaborating with the health care team. <\/span>Develops and documents plan of care from assessment data including discharge <\/span>planning in collaboration with the patient, significant others, and health care <\/span>providers. Implements interventions in a manner to maintain patient dignity, privacy, <\/span>confidentiality and safety. Defines measurable outcomes based on database and <\/span>ongoing evaluations of patient response to interventions. Identifies variances to <\/span>expected outcomes and intervenes to optimize outcomes. Involves patient, significant <\/span>others, and health care providers in the evaluation process. (i.e. Patient satisfaction <\/span>surveys, follow-up calls and CQI). Assists the patient and significant others in <\/span>identifying and securing appropriate services and educational opportunities available <\/span>to address health-related needs while providing emotional and psychological support <\/span>to patients and family. ( i.e.: Referrals, Consults, Lab and Radiology Services, <\/span>Support Groups and teaching materials). Demonstrates advanced competencies and <\/span>critical thinking skills in day to day practice. Educates staff regarding the nursing <\/span>process, and the use of clinical pathways as a collaborative practice and <\/span>interdisciplinary care tool. <\/span>30 %<\/span><\/p>

Establishes a caring and therapeutic nurse/healthcare team/patient/significant other <\/font>relationships. Recognizes communication barriers, identifies resources and <\/span>establishes plans to resolve barriers to communication. Demonstrates respect for <\/span>diversity and cultural differences in patient/significant others related to age, ethnicity, <\/span>gender, religion, socioeconomics, education, personality, or other unique <\/span>variable.Formulate individualized plans to address individual needs related to <\/span>diversity, cultural difference or other unique variables. Advocates for the <\/font>patient/significant other. Demonstrates skillful responses to customer needs based on <\/span>particular context of the situation. Provides education to staff related to the <\/span>patient/nurse relationship.<\/font>25 %<\/span><\/p>

Supports an environment of evidence based practice/research/quality improvement. <\/font>Demonstrates knowledge of Shared Governance accountabilities and principles, <\/span>supports DSGC directly as a council member or indirectly by keeping informed of <\/span>council activities/supporting council decisions, brings forth issues to council. <\/span>Demonstrates knowledge of how to access reference documents containing policies, <\/span>procedures, standards, and additional resources. Participates in QI/ Research <\/span>activities based on role and major work activities as outlined in the research point <\/span>system guidelines. Demonstrates knowledge of department QI plan. Demonstrates <\/span>understanding of resource management and implements opportunities. Collects QI <\/span>data and participates in analysis and action plan development, incorporates changes <\/span>into practice. Facilitates ongoing activities to meet and adhere to regulatory standards <\/span>(HIPAA, TJC, DFS, OSHA). Analyzes QI data and suggests action plans, <\/span>incorporates changes into practice and coaches and mentors others to do so. <\/span>Recognizes opportunities for QI processes, participates in design and implementation <\/span>of QI processes. <\/span>20 %<\/span><\/p>

Supervises other members of the team. Contributes to a supportive and healthy work <\/font>environment. Provides input into <\/span>and supports the precepting and mentoring process. Supervises select personnel in <\/span>performance/patient care. Participates in preceptor development courses. <\/span>Collaborates with other RN3s and RN4s to identify, initiate, and evaluate <\/span>teambuilding strategies. Recognized as a resource/facilitator for precepting, <\/span>mentoring, delegating, team building. Independently performs charge duties. <\/span>15 %<\/span><\/p>
