Job Description

This position is accountable to evaluate and treat all patients referred for Physical Therapy services, <\/span><\/font>to assist in their return to optimal functional abilities, and to prevent additional disability. This includes <\/span>communication with physicians and other health care personnel to assist in providing quality patient <\/span>care.<\/span><\/p>

Bachelors Physical Therapy Required.<\/span><\/font>
New graduates will be considered; experience in applicable venue preferred.<\/span><\/font>
PT - <\/span><\/font><\/font><\/span>Physical Therapist<\/span><\/font><\/p>

BLS (CPR) - <\/span>AHA Health Care Provider BLS<\/span><\/p>


Additional certifications may be preferred depending on clinical requirements.<\/span><\/font><\/p>

- If driving a Cone Health vehicle, 5 years of driving experience, MVR must be approved by Risk <\/span><\/font>Management.<\/span><\/p>


\nMAJOR WORK ACTIVITIES<\/b>:<\/span><\/font>

Completes patient evaluations, treatments, reassessments and <\/span><\/font>documentation/charging; Independently and accurately selects and performs <\/span>appropriate evaluation and reassessment techniques; Independently sets <\/span>appropriate goals; Plans and implements therapy in a manner to achieve patient <\/span>goals; Autonomously and appropriately modifies session in response to changes in <\/span>patient status, environment, and discharge plan; Independently recognizes need <\/span>for and initiates discharge from therapy services and independently recommends <\/span>and initiates referral to appropriate venue for additional therapy services; Provides <\/span>effective and thorough patient/family/caregiver education throughout the patientï¿s½ <\/span>therapy program; Supports safe, appropriate, effective and efficient patient care by <\/span>providing accurate, thorough and timely documentation that consistently meets <\/span>departmental requirements; Charges appropriately for treatment provided; Charges <\/span>are completed in a timely manner. <\/span>50 %<\/span><\/p>


Time Management: Maintains individual productivity at standard; If productivity <\/span><\/font>does not meet target, initiates and follows through on problem solving with <\/span>supervisor to increase productivity; Independently and effectively manages <\/span>caseload and other work to assure timely completion; Proactively identifies need to <\/span>change schedule throughout day to accommodate new priorities; Recognizes team <\/span>workload and proactively offers/seeks assistance to assure timely completion of <\/span>teamï¿s½ work and achievement of department and organizational goals; Efficiently <\/span>plans therapy sessions to maximize outcomes. <\/span>15 %<\/span><\/p>


Teamwork/Flexibility: Considers team priorities for the day when managing own <\/span><\/font>schedule/caseload; Consistently offers or seeks help based on the needs of the team <\/span>and overall caseload/priorities; Works with team to delegate appropriate <\/span>patients/tasks to other team members to free self for professional duties (i.e. delegate <\/span>to PTA to perform evaluations, gives tasks to front office staff to allow patient care); <\/span>Consistently utilizes non-patient care time by offering assistance to team members; <\/span>Frequently and consistently adjusts schedule throughout the day to meet department <\/span>or patient needs (related to call-outs, influx of new orders, etc.); Consistently offers to <\/span>flex schedule to support the goals/needs of the department (gives back flex days, <\/span>picks up extra weekends, works extra around holidays or during high census); Works <\/span>with team members and leadership to ensure optimal utilization of resources <\/span><\/font>including technical staff; Willingly changes rotations during the year to accommodate <\/span>team needs; For Supplemental position: works with leadership to address <\/span>department/system needs for overall caseload/priorities; frequently and willingly <\/span>adjusts schedule throughout day and on day-to-day basis to address system needs. 15 %<\/span><\/p>


Professional Communication: Consistently communicates positively and effectively <\/span>with all members of the healthcare team to support exceptional patient care; <\/span>Consistently communicates with department leadership; Communicates appropriate <\/span>information when delegating patients/tasks to other therapists, assistants, technicians <\/span>and front office staff; Maintains open communication with assistants regarding patient <\/span>needs, progress, etc. ; Provides feedback to others as indicated and in a professional <\/span>manner <\/span>10 %<\/span><\/p>


Clinical practice development: Seeks constructive feedback regarding his/her own <\/span><\/font>practice and uses this feedback to make practice changes; Assists with orientation <\/span>and training of new staff, demonstrates an ongoing commitment to improve the <\/span>performance and skill of co-workers; Serves as a preceptor to new employees on a <\/span>routine basis; Demonstrates knowledge of national standards of practice; <\/span>Incorporates these into practice and holds self and others accountable for practicing <\/span>according to these standards; Attends continuing education, reviews literature and <\/span>uses current research findings to improve and promote continual development of <\/span>knowledge; Incorporates new learning into practice; May serve as an effective clinical <\/span>instructor for students. <\/span>10 %<\/span><\/font><\/p>
